The duties of this position include but are not limited to the followingHCHV Uses knowledge of community resources to make appropriate referrals to community and other governmental agencies for services and will help coordinate services for Veterans.
Conducts psychosocial assessments and provides psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diverse backgrounds.
Incumbent uses knowledge of human development and behavior (physical and psychological) and differential influences of environment, society and culture when completing assessments and developing interventions.
Incumbent uses independent judgement and skill in utilizing supportive, problem solving, or crisis intervention techniques.
Provides counseling and/or psychotherapy services to individuals, groups, and families.
Incumbent provides social work services within the bounds of their license or certification.
Responsible for independently completing comprehensive biopsychosocial assessments, suicide risk assessments, Veteran driven treatment plans, and for providing individual and group treatments in support of mental health and housing stability.
Major responsibility of this role is to provide intensive case management services.
Incumbent provides independent social work services that are consistent with VHA Homeless Program guidelines and medical center policy (MCM) and procedures and department level standard operating procedures (SOP).
Works with vulnerable Veterans with underlying chronic medical or mental health conditions that may substantially impact Veteran's life expectancy and/or ability to function that cannot be effectively cared for due to homelessness.
Assigned Veterans may have advanced age and diminished cognitive abilities that place the Veteran at increased risk of victimization physically, mentally and/or through exploitation.
Incumbent will provide outreach to Veterans to assist with housing searches and housing retention.
Outreach will include street outreach, homeless walk in clinics, completion of homeless consults, covering homeless hotline calls, reaching out to hospital discharge planning teams, and receiving referrals from community agencies.
Incumbent will ensure that HCHV resources are targeted to chronically homeless Veterans in addition to other priority populations of homeless Veterans with the highest vulnerability.
HUD-VASH Expertise in making psychosocial and psychiatric diagnosis within their scope of practice and works with Veterans to develop measurable treatment plans to address diagnosis and identified problems.
Incumbent provides community based intensive case management services.
Provide evidence-based practice treatment to Veterans that is consistent with Housing First and Critical Time Intervention models.
Uses motivational interviewing techniques to help Veteran move forward on identified goals and will monitor progress over time.
Specialized interventions will include individual, group, and family counseling or psychotherapy and advanced practice case management that addresses complex problems associated with homelessness.
Assigned as a Mental Health Treatment Coordinator for assigned caseload and will provide case management services that help Veterans maximize health and function.
Work directly with the multidisciplinary Enhanced Team to help identify Veterans at risk of eviction and instability and will design interventions to improve outcomes.
Assist the supervisor with clinical case staffing to help determine interventions and community resources for supporting housing retention.
Responsible for maintaining a helpful and flexible approach to work and will proactively take on additional duties during short staffing levels.
Expert on VA and community resources associated with homelessness, mental health treatment, and substance use disorder treatment.
Available to the Veterans in area served and assists Veterans in linking to supportive environments in the areas in which they live Work Schedule:

GS-11 Experience and Licensure.
Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level.
OR Education.
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.
